Các yếu tố liên quan đến SDKSKHL gồm nguy cơ nhiễm Gram âm đa kháng thấp (OR = 0.33; KTC 95%: 0.13 - 0.79), CRP ≥ 6.5 mg/L (OR = 3.29; KTC 95%: 1.15 - 9.40) và số lần đổi kháng sinh ≥ 2 (OR = 10.38; KTC 95%: 3.38 - 34.72). Kết luận: Việc sử dụng kháng sinh ở người bệnh NKTNPT chưa tối ưu, đặc biệt ở nhóm bệnh có nguy cơ nhiễm Gram âm đa kháng cao, CRP ≥ 6.5 mg/L, số lần đổi kháng sinh ≥ 2.
Abstract
Background: Complicated urinary tract infections (cUTIs) are common in inpatient settings. Inappropriate antibiotic use (IAU) can prolong the length of hospital stay. Objectives: To investigate clinical and laboratory characteristics, as well as antibiotic use patterns, in patients with complicated urinary tract infections at a hospital in Khanh Hoa province. Materials and methods: A cross - sectional descriptive study was conducted on 175 medical records of cUTI patients treated with antibiotics. The appropriateness of antibiotic use was evaluated according to current guidelines. Factors associated with IAU were analyzed using logistic regression. Results: Females accounted for 62.3%; the mean age was 60.1 ± 14.7. Flank pain (39.5%) and dysuria (28.7%) were the most common symptoms. Beta - lactams, Fluoroquinolones, and Aminoglycosides were the most frequently used antibiotic classes (74.7%, 11.1%, and 10%, respectively). The overall rate of appropriate antibiotic use was 46.3%. Factors associated with IAU included a low risk of multidrug - resistant (MDR) Gram - negative infection (OR = 0.33; 95% CI: 0.13 - 0.79), CRP ≥ 6.5 mg/L (OR = 3.29; 95% CI: 1.15 - 9.40), and changing antibiotics ≥ 2 times (OR = 10.38; 95% CI: 3.38 - 34.72). Conclusions: Antibiotic use in cUTI patients is suboptimal, especially in groups with a high risk of MDR Gram - negative infections, CRP ≥ 6.5 mg/L, and those with ≥ 2 antibiotic changes.
