Abstract

Background: Vitamin K antagonists (VKAs) are commonly used for stroke prevention in patients with atrial fibrillation. The efficacy and safety of VKAs depend on the appropriateness of the indication, drug selection, and dose adjustment based on the INR. Objectives: To describe the current status of VKA use and identify factors associated with the appropriateness of treatment in patients with atrial fibrillation. Materials and method: A cross - sectional descriptive study was conducted on 156 patients. The status and appropriateness of VKA use were evaluated regarding indication, drug selection, dosage, frequency, drug interactions, adherence, and prescriber characteristics. Results: The overall rate of appropriate VKA use was 65.4%. Inappropriate use was primarily related to dosage (21.2%). Physicians aged 50 years or older or those with less than 5 years of professional experience were associated with an increased risk of inappropriate prescribing. The presence of multiple comorbidities was also a significant factor associated with the rate of inappropriate use. Conclusion: Clinical pharmacy services should focus on enhancing the individualization of treatment, particularly the adjustment of VKA dosages, and combine this with patient education on treatment adherence to improve the rational use of medication.
