Development of an evidence-based rational prescribing model for elderly outpatients

Abstract

Background: Elderly patients often suffer from multiple chronic diseases and require polypharmacy, which increases the risk of drug-related problems (DRPs). Developing a rational prescribing model to minimize DRPs is essential in the context of outpatient care in Vietnam. Objective: To develop a rational prescribing model for elderly outpatients at a hospital in Can Tho City. Methods: A cross-sectional study was conducted among outpatient prescriptions for patients aged 60 years or older in 2025. Prescription appropriateness was assessed based on drug package inserts and the Vietnam National Drug Formulary; DRPs were classified according to Decision No. 3547/QD-BYT of the Ministry of Health. In addition, international prescribing models were reviewed to establish a suitable framework. Results: Among 126 prescriptions analyzed, 53.2% contained at least one DRP. A five-step model was proposed, including clinical examination and treatment planning, pharmacist review, discussion and adjustment, patient counseling and finalization, and post-prescription follow-up. Conclusion: A collaborative physician-pharmacist model, integrated with clinical decision support software, has the potential to reduce DRPs and improve treatment safety for elderly outpatients.
