Đặt vấn đề: Phục hình cố định trên răng cối lớn hàm dưới đã điều trị nội nha thường gặp thách thức khi đối mặt với tình trạng khoảng phục hình dọc thấp, đặc biệt là đối với các răng có khoảng phục hình dọc nhỏ hơn 4 mm. Mục tiêu nghiên cứu: Đánh giá kết quả lâm sàng của phục hình dạng endocrown trên răng cối lớn hàm dưới đã điều trị nội nha có khoảng phục hình dọc thấp, theo các tiêu chí đánh giá của Cơ quan Y tế công cộng Hoa Kỳ (USPHS) trong thời gian theo dõi 3 tháng. Phương pháp nghiên cứu: Nghiên cứu lâm sàng không nhóm chứng nhóm 19 bệnh nhân có răng cối lớn hàm dưới đã điều trị nội nha có khoảng phục hình dọc thấp đến khám và điều trị tại phòng khám HIU Clinic từ tháng 12/2025 đến tháng 5/2026. Kết quả: Sau 3 tháng, 100% phục hình đạt loại A về lưu giữ, độ khít sát, bảo toàn cấu trúc và không ghi nhận sâu thứ phát; 84.2% đạt loại A về màu sắc. Mức hài lòng chung của bệnh nhân cao. Kết luận: Endocrown đạt kết quả lâm sàng khả quan trong thời gian theo dõi 3 tháng đối với răng cối lớn hàm dưới đã điều trị nội nha có khoảng phục hình dọc thấp.
Abstract
Background: Fixed prosthetic restoration of endodontically treated mandibular molars is often challenging when limited vertical restorative space is present, particularly in teeth with less than 4 mm of vertical restorative space. Objectives: To evaluate the clinical outcomes of endocrown restorations in endodontically treated mandibular molars with limited vertical restorative space according to the United States Public Health Service (USPHS) criteria over a 3-month follow-up period. Method: A non-controlled clinical study was conducted on 19 patients with endodontically treated mandibular molars presenting limited vertical restorative space who received treatment at HIU Clinic from December 2025 to May 2026. Results: After 3 months, 100% of restorations were rated Alpha for retention, marginal adaptation, structural integrity, and absence of secondary caries; 84.2% were rated Alpha for color match. Overall patient satisfaction was high. Conclusion: Endocrown restorations demonstrated favorable clinical outcomes during a 3-month follow-up period in endodontically treated mandibular molars with limited vertical restorative space.
