Chức năng cương và chất lượng cuộc sống sau phẫu thuật cấy ghép thể hang nhân tạo điều trị rối loạn cương

Đặt vấn đề: Rối loạn cương dương là bệnh lý phổ biến ảnh hưởng lớn đến chất lượng cuộc sống. Mặc dù điều trị nội khoa đạt hiệu quả với nhiều bệnh nhân, vẫn còn tỷ lệ kháng trị không đáp ứng. Cấy ghép thể hang nhân tạo là "tiêu chuẩn vàng" cho nhóm bệnh nhân này, nhưng tại Việt Nam chưa có nhiều nghiên cứu đánh giá kết quả. Mục tiêu: Đánh giá kết quả sớm về sự thay đổi chức năng cương và chất lượng cuộc sống trên bệnh nhân rối lo ạn cương được phẫu thuật cấy ghép thể hang nhân tạo. Đối tượng và phương pháp: Nghiên cứu mô tả loạt ca trên 25 bệnh nhân được phẫu thuật tại Bệnh viện Nhân dân 115 từ 2019 - 2025. Sử dụng bộ câu hỏi IIEF - 5 và thang điểm HRQoL để đánh giá trước và sau phẫu thuật 3 tháng. Kết quả: Điểm IIEF - 5 trung bình tăng từ 6.68 ± 0.99 lên 20.4 ± 1.29 (p < 0.001). Sự tự tin về khả năng cương tăng từ 1.24 điểm lên 3.96 điểm; Mức độ thỏa mãn khi quan hệ tăng từ 1.44 điểm lên 4.28 điểm. Chất lượng cuộc sống (HRQoL) cải thiện: điểm "Sức khỏe chung" chuyển từ mức Kém sang Tốt; số ngày khỏe mạnh và đầy năng lượng tăng gấp đôi (từ 9.9 lên 21.3 ngày/tháng). Kết luận: Phẫu thuật cấy ghép thể hang nhân tạo bước đầu thể hiện tính hiệu quả trong cải thiện chức năng cương dương, sự thỏa mãn và chất lượng cuộc sống cho bệnh nhân mắc rối loạn cương nặng kháng trị nội khoa.

Abstract

Background: Erectile dysfunction is a common condition significantly impacting quality of life. While medical therapy is effective for many patients, a proportion remains refractory to conservative treatment. Penile prosthesis implantation is considered the "gold standard" for these patients, but data in Vietnam remain limited. Objective: To evaluate early outcomes in terms of changes in erectile function and quality of life among patients with erectile dysfunction undergoing penile prosthesis implantation. Subjects and methods: A descriptive case series was conducted on 25 patients who underwent surgery at People's Hospital 115 between 2019 and 2025. The International Index of Erectile Function - 5 (IIEF - 5) and the Health - Related Quality of Life (HRQoL) scale were used to assess patients before surgery and at 3 months postoperatively. Results: The mean IIEF - 5 score increased significantly from 6.68 ± 0.99 to 20.4 ± 1.29 (p < 0.001). Confidence in erectile ability improved from 1.24 to 3.96 points, while satisfaction during sexual intercourse increased from 1.44 to 4.28 points. Quality of life (HRQoL) also showed marked improvement: The “General Health” domain shifted from Poor to Good, and the number of healthy, energetic days doubled (from 9.9 to 21.3 days per month). Conclusion: Penile prosthesis implantation demonstrates initial effectiveness in improving erectile function, sexual satisfaction, and quality of life in patients with severe erectile dysfunction refractory to medical therapy
