Abstract

Background: Fibrinogen is an essential coagulation factor that can be measured by the Clauss method (Fib-C) or derived from prothrombin time (Fib-Tg). Objective: To evaluate the correlation, agreement, and clinical significance between Fib-C and Fib-Tg at Tra Cu District Medical Center, Tra Vinh Province. Materials and methods: A cross-sectional descriptive study of 252 patients from December 2024 to June 2025. Both methods were measured simultaneously on the Sysmex CA-600 analyzer. Statistical analyses included Spearman correlation, Wilcoxon signed-rank test, Bland-Altman analysis, Passing-Bablok and Deming regression, and Kappa/sensitivity/specificity at thresholds of 1.5, 2.0 and 4.0 g/L. Results: Mean Fib-C was 3.82 ± 1.60 g/L versus Fib-Tg 3.56 ± 1.60 g/L (Wilcoxon p < 0.001). Spearman r = 0.998 (p < 0.001). Bland-Altman analysis revealed a systematic positive bias of +0.259 g/L (95% LoA: +0.124 to +0.393 g/L), entirely positive across all subgroups. Passing-Bablok regression showed slope = 0.998 (95%CI: 0.992-1.000) and intercept = 0.271 g/L, confirming systematic bias without proportional bias. At the < 2.0 g/L threshold: sensitivity 100%, specificity 94.9%, PPV 57.1%, NPV 100%, Kappa = 0.703. Conclusion: Fib-Tg correlates strongly with Fib-C but low positive predictive value (57.1%) at the < 2.0 g/L threshold, requiring Clauss confirmation before clinical intervention.
