Abstract

Background: Premolar extraction planning in orthodontics requires concurrent assessment of space conditions, skeletal characteristics, incisor position, and soft tissue. Objective: To compare pretreatment morphologic characteristics between patients planned for premolar extraction and non-extraction treatment. Materials and methods: This retrospective cross-sectional study included pretreatment records of 300 orthodontic patients, comprising 202 non-extraction and 98 premolar-extraction cases. Data were obtained from clinical records, facial photographs, dental casts, and lateral cephalograms; cephalometric measurements were analyzed using WebCeph. Quantitative variables were compared using the independent-samples t-test or Mann-Whitney U test according to data distribution. Results: The extraction group had greater space deficiency, smaller maxillary and mandibular arch widths, and higher TFH, LFH, FMA, and SN-GoGn values, together with greater lower-incisor and lip protrusion. OJ, OB, Xi-Pm, SNA, ANB, SE, NaPog-FH angle, Wits, U1-NA angle, and IMPA did not differ significantly. Conclusion: Premolar extraction decisions were associated with multiple dental, skeletal, and soft-tissue characteristics. Treatment decisions should therefore be based on comprehensive individual assessment rather than on a single parameter.
