Abstract
Background: De Quervain syndrome is a common wrist disorder; however, its pathophysiology and treatment response remain incompletely understood due to the anatomical variability of the first extensor compartment. Objectives: To analyze the role of anatomical variations, the value of ultrasound, and the trend toward personalized treatment in De Quervain syndrome. Material and methods: This narrative review was conducted through a systematic literature search of PubMed, Scopus, and Web of Science. Relevant clinical, anatomical, and imaging studies on De Quervain syndrome were included. Results: The first extensor compartment demonstrates substantial anatomical variability, particularly regarding intracompartmental septation and tendon number. Ultrasound plays an important role in detecting these variations and in guiding treatment stratification. Anatomical factors significantly influence the likelihood of corticosteroid injection failure and inform surgical decision-making. Conclusion: De Quervain syndrome is a more complex entity than traditionally perceived. A personalized approach integrating clinical assessment and ultrasound findings has the potential to improve treatment outcomes.
